This report offers the most recent advances in the field of survivorship. The report’s strategies, successes, challenges, knowledge, tools and resources can help implement care plans and projects.
This report presents the Partnership’s work about survivorship to date, including the following projects:
- Developing an evidence-based guidance document on survivorship care
- Implementing and evaluating four demonstration projects about survivorship care plans
- Creating several support programs for survivors and their families
- Investigating cancer survivors’ workplace issues
Cancer Chat, formerly Cancer Chat Canada, is another survivorship resource funded by the Partnership in 2015. The site provides professionally-led online support for Canadians affected by cancer.
The Partnership’s Person-Centred Perspective initiative is committed to improving the patient experience. We are working with partners across Canada to find the best ways to offer a person-centred perspective throughout a person’s cancer journey and to help information flow throughout. The impact of a cancer diagnosis goes far beyond the physical disease. It affects every aspect of a person’s life. The initiative has focused on reporting about the patient experience, and giving health-care providers patient-centred tools and resources, which have been validated and standardized.
